On the day of surgery, you will meet initially with the nurse and then the anesthesiology staff, who will carefully explain the type of anesthesia they will be using. Most patients do not have a significant amount of pain during this time and we recommend taking pain medications as needed every four to six hours. Facelift surgery is commonly performed to correct sagging along the jaw-line (known as jowls) and neck region (commonly referred to as the turkey-neck). Progressive drooping of our facial skin and muscles causes these deformities. Traditionally a facelift would involve tightening only the skin component of the face and neck. The biggest drawbacks of skin only facelifts are somewhat unnatural appearing results with a “pulled back look”, poor scars, and shortly lived results. More advanced facelift techniques involve repositioning the deeper layer of tissue, known as the SMAS (Superficial Muscular Aponeurotic System), which tightens the muscular layer beneath the skin. The benefits of a SMAS facelift are longer lasting results, a more natural appearance, and minimal scarring. Additionally, when a SMAS layer is used in a facelift, it can be repositioned separately from the skin, allowing us to perform a bidirectional facelift. This serves to correct different deformities, and provides optimal results. Due to the complexity of a SMAS facelift, many surgeons are not comfortable performing this type of procedure, and nor do they have the appropriate training to do so. In addition to his training in plastic surgery, Dr. Kapoor has had an additional year of fellowship training in SMAS facelift surgery by Dr. John Owsley, one of the pioneers of the SMAS facelift. Male Breast Reduction Post Operative Instructions

1. You must have a responsible adult drive you to your plastic surgeon on the day of Male Breast Reduction Surgery, drive you home after Male Breast Reduction Surgery and care for you for 24-48 hours after Male Breast Reduction Surgery.

2. Early ambulation is important after Male Breast Reduction Surgery; you may start walking lightly 1 day after your surgery.

3. You may go to the bathroom with assistance as needed.

4. Take your medications according to the specific written instructions that have been provided by your cosmetic surgeon.

5. You can expect the following:

You will feel significant pressure on your chest. This will resolve after a few days time.
You will be fitted with a post surgical garment and foam compression pads to give you added support. This garment usually has to be worn night and day for 5 weeks after surgery, except when showering or when laundering it. It is not uncommon to have some mild bloody drainage on your garment the first day.
A drain sometimes will be in placed in each breast and the drainage will need to be measured with the amount recorded and then emptied. It will usually be removed at your first postoperative visit 3-4 days after surgery. At this time, sutures will be removed.
The period of greatest discomfort does not usually last more than 24-48 hours. Thereafter, you may find that you need your pain medication less frequently.

6. You can shower 48 hours after Male Breast Reduction Surgery. Remove the garment and discard any gauze dressings and foam pads from the chest. Do not remove the surgical tape from the incision. After the shower, replace the garment.

7. Avoid any heavy lifting (greater than 10 pounds), strenuous exercise, pushing for 2 weeks after Male Breast Reduction Surgery. After this, you can gradually increase your activity, progressing to full activity with no restrictions at 3 weeks.

8. Avoid direct sun exposure / tanning of the incision for several months after Male Breast Reduction Surgery.

9. A liquid diet is recommended for the first day progressing to a regular diet over the first 24 hours.

10. Do not drive a car until your cosmetic surgeon has given you permission to do so.

11. Avoid smoking and alcohol for 2 weeks after Male Breast Reduction Surgery.

12. Contact your plastic surgeon if you experience any of the following:

Severe pain not responding to the pain medication.
Marked swelling of one or both breasts, fevers, chills, or excessive bleeding.
Shortness of breath, chest pain.

After the surgery is complete, a bandage may be loosely applied to the head and a drainage tube may be placed under the skin behind the ear. The drainage tube will be removed within one to two days after surgery, and the bandage within one to five days. The face is likely to appear pale, bruised and puffy at first, and the skin may feel tender and numb. The numbness should fade within several weeks to months, while the bruising should subside within about two to three weeks. Keeping the head elevated as much as possible during the first few days after surgery will help to alleviate the swelling. Patients may experience minor discomfort which can be controlled with prescribed pain medication. Patients should plan on taking it easy for the first week.

After liposuction surgery, a small drainage tube may be inserted at the surgical site and this will be removed within a few days after surgery. Bruising and swelling are common after liposuction but usually subside within three weeks. Some minor swelling may last for up to six months. Patients will need to wear a special elastic garment similar to a girdle for several weeks as this will help to control swelling and support the new contours of your body. Patients may also experience some discomfort, burning, itching or temporary numbness. Discomfort can be controlled with prescribed medication. Significant improvement may be seen within four to six weeks after the liposuction procedure, with the final result apparent after about three months.

Tears pass through the eyelid puncta and enter the superior and
inferior canaliculi oriented at right angles to the lid. The vertical
portions of the canaliculi course for 2 mm before turning at
right angles and moving parallel to the lid for 8 mm. There is
some variability in the way that the superior and inferior canaliculi
unite. They may join the sac separately or converge into a
common canaliculus. At this junction lies the valve of Rosenmüller,
a flap of mucosa that prevents reflux of tears back into
the canaliculi.

The inferior end of the lacrimal sac tapers to form the lacrimal
duct as it enters the nasolacrimal canal formed by the
maxillary, lacrimal, and inferior turbinate bones. The NDL
runs within this osseous canal for a distance of approximately
12 mm. It continues beneath the inferior turbinate as a membranous
duct for an additional 5 mm before opening into the
inferior meatus. The duct orifice is found at the junction of the
middle and anterior third of the meatus, approximately 8 mm
behind the anterior tip of the inferior turbinate and 29 mm from
the anterior nasal spine. It is often covered by a flap of mucosa,
known as Hasner’s valve, which is thought to prevent reflux of
nasal secretions. Gentle pressure over the medial canthal region
will often produce fluid or bubbles at the duct orifice to confirm
its location.

Tummy Tuck Post Operative Instructions

1. You must have a responsible adult drive you to your plastic surgeon on the day of tummy tuck surgery, drive you home after tummy tuck surgery and care for you for 24-48 hours after tummy tuck surgery.

2. Early ambulation is important after abdominoplasty; you may start walking lightly 1 day after your abdominoplasty surgery.

3. You may go to the bathroom with assistance as needed.

4. Take your medications according to the specific written instructions that have been provided.

5. You can expect the following:

You will need to remain in a flexed position when lying down and will likely feel as you if you cannot stand straight for several weeks after tummy tuck surgery. This will resolve after a few weeks time.
For the first 24-48 hours, you should sleep on your back with pillows behind your head and under your knees to relieve pressure from your lower back.
A drain will be in place and the drainage will need to be measured with the amount recorded and then emptied. It will usually be in place 10 to 14 days.
You will be fitted with an abdominal binder or garment to give you added support. This binder usually has to be worn night and day for 5 to 6 weeks after tummy tuck surgery, except when showering or using the bathroom or for laundering it. It is not uncommon to have some drainage on your garment or binder.
The period of greatest discomfort does not usually last more than 24-48 hours. Thereafter, you may find that you need your pain medication less frequently.

6. You can shower 48 hours after tummy tuck surgery. Remove the binder and any gauze dressings from the incision and belly button area. Do not remove the surgical tape from the incision. Clip the drain to a belt or string around the neck or waist. After the shower replace the binder or garment.

7. Avoid any heavy lifting (greater than 10 pounds), strenuous exercise, pushing for 5 weeks after tummy tuck surgery.

8. Avoid direct sun exposure / tanning of the incision for several months after tummy tuck surgery.

9. A liquid diet is recommended for the first day progressing to a regular diet over the first 24 hours.

10. Do not drive a car until your physician has given you permission to do so.

11. Avoid smoking and alcohol for 2 weeks after tummy tuck surgery.

12. Contact your plastic surgeon if you experience any of the following:

Severe pain not responding to the pain medication.
Marked swelling, fevers, chills, or excessive bleeding.
Shortness of breath, chest pain.

Breast implants for augmentation may be placed via various types of incisions:

Inframammary - an incision is placed below the breast in the infra-mammary fold (IMF). This incision is the most common approach and affords maximum access for precise dissection and placement of an implant. It is often the preferred technique for silicone gel implants due to the longer incisions required. This method can leave slightly more visible or thicker scars.

Periareolar - an incision is placed along the areolar border. This incision provides an optimal approach when adjustments to the IMF position or mastopexy (breast lift) procedures are planned. The incision is generally placed around the inferior half, or the medial half of the areola's circumference. Silicone gel implants can be difficult to place via this incision due to the length of incision required (~ 5 cm) for access. As the scars from this method occur on the edge of the areola, they are often less visible than scars from inframammary incisions in women with lighter areolar pigment.

Transaxillary - an incision is placed in the armpit and the dissection tunnels medially. This approach allows implants to be placed with no visible scars on the breast, but is more likely to produce asymmetry of the inferior implant position. Subsequent revisions of transaxillary-placed implants usually require inframammary or periareolar incisions. Transaxillary procedures can be performed with or without an endoscope.

Ear Surgery ( Otoplasty ) : The Procedure Described

Ear Surgery is usually performed under general anesthesia in children. Adults may be more likely to have ear surgery under local anesthesia with sedation that produces a twilight sedated effect in a semi-conscious state. In some cases of general anesthesia, the patient remains overnight at a hospital for observation. The procedure may be performed at our state of the art facility or an affiliated hospital. The procedure requires up to fours or more of time, depending upon the technique required to perform the procedure.

There are a variety of techniques that may be used for ear surgery. We select the technique that is most suitable for the patients’ particular situation. Often, ear surgery entails an incision made behind ear in the natural crease of the juncture between the head and ear. Then, excess skin and cartilage can be reshaped or removed. Finally, the cartilage and skin is sutured back into place. In other cases, the cartilage does not need to be trimmed or removed, allowing for simple reshaping of the cartilage through suturing that will hold the newly refined ear in its proper position. Then, the patient will be bandaged. Ear surgery is usually performed on both ears, regardless of only one ear affected, to produce the most symmetric results. Ear surgery patients will be bandaged to help hold the ears in place.

Ear Surgery Recovery

Ear surgery patients are usually up and around with a few hours following surgery. Within a few days thereafter, the large bandaging is removed and replaced with dressings. Stitches are removed within a week after surgery. Children can usually return to school within a week of surgery and adults require five days. Care must be taken to avoid activities that may involve the ears for at least a month following ear surgery.

Complications are very rare in ear surgery and can be medically managed, including infection, excess scar tissue, and blood clot. Patients are usually satisfied with the aesthetic results of ear surgery and enjoy more productive lives following ear surgery.

Recovery Following Face Lift Surgery in Los Angeles

Facelift surgery patients usually do not experience significant discomfort following surgery. Pain medication may be used, if required. Patients usually return to work within five or more days of the procedure. For the first few days after surgery, you will be directed to follow post operative instructions which may include, but not be limited to:

* Keep the head in an elevated position
* Refrain from activities that put pressure on the head such as bending over and exercise
* Do not go in the sun, use SPF 15 sunscreen thereafter
* Follow all other post operative instructions provide to you

While the risk of complications is very low, it is important to report any possible complications to our office and seek treatment. Most complications can be medically managed when treated in a timely fashion.

The Mini Facelift, the Micro-Lift, and the Standard Facelift

There are three types of facelifts that Dr. Kapoor performs: the mini facelift, the micro-lift, and the standard facelift. All involve deeper layer techniques involving the SMAS layer and bi-directional repositioning of the skin and SMAS layer. The choice of which procedure to perform will depend on patients goals and anatomy. After a consultation with Dr. Kapoor and his staff, we will be able to appropriately recommend a treatment plan.

For patients in their late 30’s to early 40’s, a mini facelift is an appropriate treatment that will address sagging of the cheek and along the jawline. This usually involves a small incision along the front of the ear, through which the cheek and sagging tissue are lifted. This procedure takes approximately 1 and a half hours and is performed under local anesthesia with intravenous sedation or general anesthesia if preferred.

For patients in their 40’s to early 50’s, a micro-lift will correct sagging of the cheek, the jawline and neck. This usually involves a small incision along the front of the ear that into groove behind the ear, through which the cheek, jawline and neck tissues are lifted. A micro-lift provides the same benefits of a full facelift, but with smaller incisions and shorter recovery period. This procedure takes approximately 3 hours and is performed under local anesthesia with intravenous sedation or general anesthesia if preferred.

For patients with significant neck laxity and excess skin, a standard facelift will address all the visible signs of aging, but with a slightly longer incision that extends along the posterior hairline. This procedure takes approximately 3 and a half hours and is performed under local anesthesia with intravenous sedation or general anesthesia if preferred.
